Questionnaires are used in various modules in Evalea :

To create a questionnaire, you can find the option either module-specifically within the main menu or within the corresponding AddOn or detailed view:

questionnaire overview

  1. ID : Each questionnaire is automatically assigned its own ID. Editing the questionnaire automatically creates a new ID.
  2. Name : Specifies the name of the questionnaire.
  3. Evalea Standard Questionnaire : Indicates whether this is a pre-installed Evalea questionnaire.
  4. Audience : Specifies the audience selected during creation.
  5. Theme : Specifies the theme selected during creation.
  6. Created : Indicates the creation date of the questionnaire
  7. Creator : Indicates the name of the creator of the respective questionnaire.
  8. Available for : Indicates which roles this questionnaire is available for.
  9. Options : From left to right: Magnifying glass : Preview the questionnaire, Gear : Edit the questionnaire, Delete : Delete the questionnaire, Set access : Specifies which role is allowed to use this questionnaire.

Creating a questionnaire

  1. Name : Specifies the name of the questionnaire.
  2. Topic : Optionally specifies a topic assignment.
  3. Target group : Optionally specifies a target group (participant, manager).
  4. Add : Creates the questionnaire. You will be redirected to the questionnaire editor.
  1. Start with a template : Allows you to copy and edit existing questionnaires.
  2. Delete : Deletes the created questionnaire
  3. Mark as draft : Questionnaires marked as draft cannot be used for surveys, evaluations, etc. Clicking the icon again deactivates the draft status.
  4. Name : Edit the name as needed.
  5. Plus sign : Adds question items.

  1. Specifies the size of the answer text field
  2. Ask your question here. This applies to all question types.
  3. Marks the question as mandatory. The questionnaire cannot be sent until all mandatory questions have been answered. This applies to all question types.

Define labels and values:

  1. Describe the individual scale points
  2. Enter a value (positive, whole numbers) for each scale point. This enables an evaluation.
  3. Choose whether the values should be displayed behind the scale points or not.
